[H-GEN] Exim + Mailscanner + Spamassassin problems

I'm running two instances of exim on Debian. One pushes mail to mailscanner's 
work directory, mailscanner scans it, then the other delivers outgoing stuff. 
It works as described on the mailscanner site. However I'm loosing lots of 
mail. People get errors like this:

Does anyone know why that would occur and how I can stop it? Thanks,
