So having tried 2 MTAs we could consider ruling that out as a possible
cause (although we can't 100% rule it out yet).

How is performance with ftp?

I suspect you will see problems with this, and that is won't be mail
alone.  If this is the case, I can offer a few suggestions as to possible
problems.  Shouldn't be too hard to fix at that point.

I don't think masq is at fault, but supply us with the ipfwadm commands
you are using anyway.
