No idea.  But the following may help:

1.  The contents of squid/access.log for the fetch that
    failed.

2.  The error message returned by Windows.

3.  The output of the following command while executing
    when the windows box fails:

    tcpflow -i eth0 port 3128

    (where eth0 is your internal lan interface).
