I don't recall whether this possibility was discussed, but are you running 
samba as a daemon or using tcpwrappers?

Could the issue be something on the server trying to do reverse lookups of 
the IP address, or engaging in some other anti-spoofing activity? How long 
does it take on the server to do a reverse nslookup for the ips of hosts 
connecting to it? Does the lookup succeed or fail?
