I would add:
Maintenance:
Is the source code under an acceptable
license? Will you or someone you know
and trust be able to hack it to fit
your purposes if necessary?
Does the author or some other person still
maintain the code to keep it up to
date with standards and security
issues? Are there regular updates
from them or your distribution vendor?
How do they respond to bug reports and
feature requests?
Is there a mailing list/newsgroup/whatever?
(Do they eat their own dog food?)
