The FQDN (fully qualified domain name) should always be first in the
/etc/hosts file.
In addition, duplicate entries are bad (tm).
Change it to:
127.0.0.1 localhost
203.45.199.87 caffeine.dyn.dhs.org caffeine securemail.2y.net securemail
