I was always under the impression it would be a 2 page PDF where
people who had a duplexer could print double sided or 2 pages for
people without. Then even people having issues with double sided
printing can still print 2 pages.

The main reason for creating a new form is to make sure we get the
details HUMBUG needs on record and to add the updated logo. Also, the
exec does not seem to have the original document the membership form
was printed from as what we are using is a few generations removed
from the original through photocopying.
That said, if any former exec member may be able to help by pointing
us in the right direction if the original document exists, we would be
most grateful.
