Thank you very much!!! How long has that been the case? As I said I 
looked very hard to find the solution but obviously I looked in the 
wrong places, including this page:

http://www.squid-cache.org/Doc/FAQ/FAQ-17.html

As you can see it still advises to use those httpd_accel options.

I made the change you suggested and struck this bug:
http://www.squid-cache.org/bugs/show_bug.cgi?id=1650

As it says in the notes the fix is to add to squid.conf the line:
always_direct allow all

I have transparent proxying working again!!!!!
