Yeah, OK, I skipped a step.  (It's been a little while).

You *do* need to convert the video into a raw MPEG stream, and you *do*
need to convert the audio into a raw MPEG stream in a separate file.  I
stuffed up by saying that these files are what you need for a VCD.  At
this point you need to multiplex the files together into an MPEG system
stream (as Chris said) which can then be used for the VCD.  This is done
with either "mplex" from Mjpeg tools or "tcmplex"  from Transcode.

Again, I like mplex from Mjpeg tools.  It will create files compatible
with VCD and SVCD (I should check how DVD output is going, and XVCD).  It
also has options for variable bit rate multiplexing.
