There are several approaches.  One large one is using milter in
combination with sendmail (insert method for your MTA here).
Another is to use procmail.

Spamassassin can be configured to recognise this condition via
a custom rule.  Add the following to the preferences file and
experiment:

    header FOO_BAR_TEST ALL =~ /cool/i
    score FOO_BAR_TEST 50.00
    describe FOO_BAR_TEST Testing...
