I'm currently using Mailscanner with Sendmail. It marks possible spam as 
spam, as well as allowing the blocking of possibily hostile executables, 
naughty javascript and can feed files through another virus scanner. 

Openantivirus is GPL project that is developing a GPL virus scanner. The 
signatures aren't updated all that often, and at this point it still isn't 
supported by Amavis or Mailscanner, but it's still under development, and 
nowhere near beta yet. It can also plug into Samba to scan files, and do 
other interesting things.

So between rolling your own stuff in procmail, or these programs you should 
be able to come up with a solution. Hope this helps. Thanks,
