I don't know about an online source, but I have borrowed a book from the
library which covers just this in a section.

"Official Guide to Programming with CGI.pm"  Lincoln Stein.  page 149 file
uploads.

If necessary, I could scan in the few pages about it and post them
somewhere for you to view.
