This may be stating the obvious but have you got 3 other hard drives.
setting up the CD on the secondary slave is not recommended even under
DOS, so if you don't have three IDE hard drives you should be setting up
as hdb or hdc.

Further, what is the jumper setting on the CD, DOS and Windows are
pretty tolerant but Unices are not so. If you set up on master the
jumper should be set to master and if slave the jumper should be set to
slave.
