Michelle,

I rang HSW as suggested, and their main concern is every box is filled 
in on page 2.  In the attached document I've filled in a few more on 
page 1 to cover my bases.  The ones they mentioned where:

Q: What arrangements will be in place for cleaning of the event space, 
facilities and equipment?
?: We always clean up, returning it to how we found it.  We can wipe it 
over if required, but I'm guess UQ cleaning comes through as well?

Q: Endorsed by Executive Dean or relevant USMG member, y/n, name, 
position, signature.
?: I'm not sure what to put here - is No an acceptable answer?  If not, 
the others have to be filled in by ITEE.

could please provide additional information on page 2 of the PF700 form 
attached, (Health, Safety and Wellness (HSW) are seeking further detail 
around asking the health screening questions on the day).
