[H-ADMIN] Meeting Template

Hi Guys,

I'd like to suggest that we adopt the minutes template from the HUMBUG
wiki at http://www.humbug.org.au/Exec/MinutesTemplate for all our past
and future exec meetings. This was created by James not long after the
AGM.
I have now modified so that it better fits our needs. 

The main things I'd like to point out with this template is that the
agenda is the automatically generated contents section of the page, not
any of the items below that. This means that the Agenda items are kept
separate from the main minutes. This was something that Raymond seemed 
to have an issue with when James first tried to implement the Minutes
Template.

I've also proposed a motion that we have at the start of each meeting.
This is to take all discussions that occur on the wiki prior to the
meeting as having occurred at the meeting. This is similar to how Linux
Australia operates their council meetings except they take entire items
as written where as we are just taking input into the discussion as
written. All of our voting would still occur at the meeting. 

The idea behind this is that any Humbug member can put forward there own
agenda items and have a record of the following discussion on the wiki.
By having a template that is easy to follow it allows for easy insertion
and removal of agenda items. 
This would serve to aid participation of Humbug members in the Exec
meetings. Making the Exec more open and accessible to members. 

The minutes can also be written in live form as we had with the AGM or
notes taken down to be added in later (these could be added as
attachments in the wiki). Either way it forces the minutes to be clear
and readable.
 
Another advantage to having this template would be that it would stop
excessively long arguments and discussions as these would have been done
before the meeting. Hopefully shortening the meeting and creating more
productive outcomes.
